Grant from the Mother Cabrini Heath Foundation will support the expansion of care management services to families with children and older adults

Maimonides Medical Center, in partnership with Maimonides-managed entity the Brooklyn Health Home, today announce that they have been awarded $500,000 from the Mother Cabrini Health Foundation to expand care management services.

The funds will support the training of pediatric and geriatric care managers, bringing crucial resources and supports to two key groups – families with children and older adults. These care managers will make referrals to social services, connections to programs, and facilitate engagement with a variety of primary, specialty, and behavioral health services.

Through these specifically trained care managers, Maimonides Health and The Brooklyn Health Home will advance health equity, reduce barriers to care, and address social factors impacting health for at-risk individuals in Brooklyn.
