Services

The Brooklyn Health Home was established to coordinate medical and behavioral healthcare services for Medicaid patients. It is built on the foundation of a successful program developed at Maimonides, along with many community-based organizations with funding provided by New York State.

Eligibility

If you are a Medicaid recipient or believe you may qualify for Medicaid, think about the following questions:

  • Do you have chronic medical or mental health conditions for which you need regular doctor’s care?
  • Do you have a doctor you can see when you need to?
  • How many times have you been in the emergency room or hospital in the past six months? Twelve months?
  • Do you have a safe place to live?
  • Do you have someone in your life to help you whenever you need help?
  • Do you have difficulty keeping medical appointments? 
Brooklyn Health Home will connect you with
  • Health care providers
  • Mental health and substance abuse providers
  • Medications
  • Housing services 
  • Social services (food, benefits, and transportation)
  • Other community programs that will support and assist you.
Beyond Health Home – CCB Navigator

The CCB Navigator connects Brooklynites to a network of partner organizations that provide the services and resources you need to stay safe, access quality services and avoid health crises. The Navigator is free and available to all Brooklyn residents through a referral or a direct call.
