Maimonides Medical Center Partners with Unite Us to Launch CCB Navigator

The CCB (Community Care of Brooklyn) Navigator is slated to launch in early 2022 and will transform care management coordination in Brooklyn.

Maimonides Medical Center, Brooklyn’s largest hospital, and The Brooklyn Health Home, a Maimonides-managed entity that provides community-based care management services to individuals living with multiple chronic illnesses, are partnering with Unite Us, the enterprise technology company providing the digital infrastructure to power integrated health and social care ecosystems, to launch a transformative coordinated care network in Brooklyn and expand the existing network known as Unite NYC

This community-based approach to develop a secure coordinated care network aims to provide patients with the resources they need to stay safe, access quality services, and avoid health crises. The CCB Navigator will serve as a single touchpoint to connect Brooklyn residents with mental and behavioral health care and coordinate connections to social services.

Utilizing a person-centered, custom needs assessment and matching process to streamline referrals, the system will coordinate from within the Maimonides Medical Center system, Brooklyn Health Home’s network of community-based partners, and the Brooklyn community to connect patients directly to services and assess for care management needs at every point in a patients’ care journey.

“The CCB Navigator will allow us to more efficiently and directly screen referrals and connect vulnerable members of our community with organizations providing behavioral health and social support services tailored to their needs while respecting their priorities,” said Magdalena Gordon, Executive Director of The Brooklyn Health Home. “We know how important it is to address behavioral health challenges and social determinants of health to improve health outcomes and overall quality of life, yet navigating the terrain of health care services can be incredibly difficult for patients. We are pleased to be partnering with Unite Us to provide comprehensive and accessible assistance to Brooklynites with complex care needs.”

Community partners, health systems, plans, and governments across the country utilize Unite Us’ secure and HIPAA-compliant platform to coordinate care. With robust protection and certifications aligned with industry-leading privacy and security frameworks such as HITRUST, Unite Us makes it possible for providers to improve the health of their clients and community while maintaining privacy and trust.
